: The study received ethical approval from Psychiatry, Nursing & Midwifery Research Ethics Subcommittee, King’s College London, UK (reference number: PNM/10/11-124), and the Faculty of Medical Sciences University of Sri Jayewardenepura Ethical Review Committee (USJP ERC) (reference number: 596/11). Ethical principles were followed by collecting written informed consent from all respondents, for each study component they opted to participate in.
