: The Institutional Review Board of Institute of Tropical Medicine, Antwerp, Belgium and the Ethics Committee of Nepal Health Research Council, Kathmandu, Nepal, provided the ethical approval. Study consent was also provided by National Center for AIDS and STD Control, Kathmandu, Nepal and District Health Office, Achham, Nepal.
