: MK is a community health nursing lecturer and doctoral student at the School of Nursing, College of Health Sciences, University of Nairobi; WM and SK are senior lecturers at the same school, University of Nairobi.
: Ethical approval of the study protocol was sought and granted by Kenyatta National hospital/University of Nairobi Ethics and Research Committee (Ref: KNH-ERC/A/90 protocol number – P706/11/2015). Authority to conduct the study was obtained from Kiambu County, Lari subcounty authorities and all health facilities involved. Participation in the study was purely on voluntary basis. All the study participants provided verbal and written informed consent before recruitment into the study. The participants who were under-age (below 18 years of age, since legal age in Kenya is 18 years) are considered as emancipated minors who can make independent decisions. The minors who agreed to participate in this study provided signed informed assent and their parents provided an informed verbal consent on their behalf. The minors were included because in Kenya, the percentage of women who have begun childbearing increases rapidly with age, from about 3% among women age 15 years to 40% among women age 19 years [].
