: The study has been approved by the research ethics committee of the University of Hong Kong and the Hong Kong West Cluster of the Hospital Authority (IRB reference no.: UW16–320). The trial was registered at ExternalRef removed as NCT02804880. The study poses a minimal additional risk to study participants.
