Ethics approval and consent to participate

: The project was approved by the Data Protection Officer at the Norwegian Institute of Public Health. As the Survey was anonymous no written consent from the IRB was needed according to Norwegian law (Directive 95/46/EC, paragraph 26). An introductory statement to the survey made it clear that participation was voluntary.
