: The FPS study was approved by the Ethics Committee of the Helsinki and Uusimaa Hospital District. Answering to the FPS survey was voluntary and the completed questionnaire acted as an informed consent. The JAGES was by the Ethics Committee for Research on Human Subjects at Nihon Fukushi University, Japan (No. 10–05).
