: For the Finnish study (TAM), the study protocol was approved by the Ethics Committee of the National Institute for Health and Welfare (formerly National Public Health Institute). For the Swedish study (NoSCo), study protocol was approved by the Regional Ethics Vetting Board in Umeå. Participants were informed of the purposes of the study and that participation was voluntary. They were requested to indicate their consent by answering the survey questionnaire.
