Ethics approval and consent to participate

: All participants approved consent, and for participants below the age of 18 years, written parental consent was required before they were allowed to fill in the questionnaire. The study was conducted in accordance with the Declaration of Helsinki. Ethical guidelines were followed, privacy of all respondents was guaranteed and all data were treated anonymously. According to the Ethics Committee Bèta-Geo of Utrecht University, ethics approval was not required given the facts that the study concerned was not medical-scientific and the respondents were not subject of actions or rules of conduct; requirements that must be met if research needs ethics approval according to Central Commission of People-oriented Research (CCMO).
