: The Charles Darwin University Human Research Ethics Committee granted ethical approval for this study (Phase 1 H12136 and Phase 2H16025). Written consent was obtained from all participants following in-depth explanation about the study in the participant’s primary language to ensure consent was fully informed. Written consent from a parent or guardian was obtained for participants aged under 18.
: Participants and parents/guardians of children who participated were given the opportunity to chose how they wanted to be identified in publications. These choices were recorded on written consent forms and all chose to use cultural group names that do not identify specific individuals. These preferences were again confirmed when the manuscript was completed.
