Ethics approval and consent to participate
: According to the Regional Committees for Medical and Health Research Ethics (REC South East, 2016/1289), ethical approval was not necessary. Nor was there a need for approval from the Norwegian Data Inspectorate, as the data collection was anonymous, was not directed toward patients and did not collect person identifiable or sensitive data. All participants were given written information about the study, and submission of the completed questionnaire was considered consent to participate.
