Ethics approval and consent to participate
: The study was approved by the Research Ethics Committee of Royal College of Physicians of Ireland December 2013 (RCPI RECSAF 20).An extensive participant information leaflet was included with the questionnaire outlining the purpose and scope of the study (Additional file InternalRef removed). The leaflet also explained that the information was being gathered anonymously to ensure confidentiality and that the response to the questionnaire was indicative of consent. Specifically, potential participants were informed that “by responding to the questionnaire either electronically or by post, you are consenting to participate.” This method of consent was approved by the Research Ethics Committee of Royal College of Physicians of Ireland December 2013 (RCPI RECSAF 20).
