Ethics approval and consent to participate
: We submitted and obtained from ethics approval from Public Health Ontario Ethics Review Board, and from case study sites where required, including the Toronto Public Health Research Ethics Board, Fraser Health Research Ethics Board and Simcoe Muskoka District Health Unit Ethics Review Board. Ethics approvals were subsequently renewed annually. Case study team member and key informants were consented for each data collection activity as each one took place. For logistical reasons, including scheduling of 1:1 interviews across diverse sites, verbal consent was recorded for the semi-structured interviews as approved by each ethics review board. Written consent was obtained for the online surveys, passive team observation, and data interpretation meeting. We also completed and submitted Privacy Assessments that described different types of potentially identifying information we collected, how we de-identified data for reporting and dissemination, and the security measures put in place to protect all collected data. Case study site names and identifiers were coded with a letter and/or a number to protect confidentiality.
: During study participant recruitment and consent, we outlined that all study results will be aggregated and de-identified. Study participants consented to publishing study results.
