: The Senate Research Committee of the University of the Western Cape (ID NO: 15/5/5), South Africa and the Ethical Review Committee of the Ghana Health Service (ID NO: GHS-ERC: 01/07/13), Ghana, approved the study. Additional approval was obtained from the Municipal Education Directorate of the Ghana Education Service, and from the heads of participating schools while parents or legal guardians of children provided written informed consent.
