: This study was conducted according to the guidelines laid down in the Declaration of Helsinki and all procedures involving human subjects were approved by the Ghana Health Service Ethics Review Committee (Protocol number GHS-ERC:07/09/2016), for primary data collection and an ethics checklist was approved by the University of Sheffield.
