Ethics approval and consent to participate

: The study protocol was reviewed and approved by the Stanford University Institutional Review Board (IRB, Protocol ID: 34706), as well as the Kenya Medical Research Institute (KEMRI) Scientific and Ethics Review Unit (SERU, #NONSSC515). Because study subjects were under the age of 18, written informed assent was required. Written, opt-out informed consent was also sought from parents.
