Ethics approval and consent to participate
: Ethics approval for the project was obtained from both the Conjoint Health Research Ethics Board at the University of Calgary (Ethics ID: REB15-0629_REN2) and the London School of Hygiene and Tropical Medicine MSc Research Ethics Committee (Ethics ID: 10732). Waiver of consent for this study was received from the University of Calgary Conjoint Health Research Ethics Board (Ethics ID: REB15-0629_REN2) due to the impracticality to contact a substantial proportion of the population in the Calgary Health Region to ask consent for access to their secondary data after a considerable time has elapsed, which is justified given the negligible risk this research poses.
