Ethics approval and consent to participate
: This study received ethics approval from the Monash University Human Research Ethics Committee (MUHREC) on 8 October 2014 (CF14/2995–2014001663), including approval of the verbal consent process. Informed consent was obtained from each participant via telephone interview prior to the administration of the study questionnaire, and following delivery of an information booklet to participants via post approximately two weeks prior to telephone contact.
