Ethics approval and consent to participate
: The ethical review committee of the University of Fort Hare and Ondo State Ministry of Health, Nigeria approved the study protocol. All participants provided signed written informed consents after their confidentiality and anonymity were guaranteed. For the few participants (<i>n</i> = 55) included in this study who were aged 17 years at the time of the survey, assent and parental consent were obtained before their participation.
