Ethics approval and consent to participate
: The study was granted ethical approval from the School for Policy Studies, University of Bristol. The trial was registered with Internal Standard Randomised Controlled Trial Register under the reference number ISRCTN34001941. Written consent was obtained for all participants. Written parental consent was obtained for child participants. All participants received an information sheet detailing that their participation was voluntary, their information would be used for research purposes only and treated confidentially and that they could withdraw from the study at any time.
