Ethics approval and consent to participate
: The study protocol of the two surveys in Malemba-Nkulu and Kayamba was approved by the Ethical Review Board (ERB) from the Saint-Antoine University Hospital in Paris (CPP SA 16106) and by the Lubumbashi University (UNILU/CEM/060/2016). The study protocol of the two surveys in Manono and Fungurume was approved by the ERB from Kinshasa University (ESP/CE/126/15), the Lubumbashi University (UNLU/CEM/061/2016) and the MSF ERB.All MoH and administrative authorities authorised this study at different levels including the central office of the HZs and the village chiefs. We obtained a signed informed consent statement from each household head before starting the interviews. Signed informed consent was also obtained from the parents of the children (age 6 months to 14 years old), if parents were different from the head of the household.
