Ethics approval and consent to participate

: The study was approved by the Biomedical & Health Sciences, Dentistry, Medicine and Natural & Mathematical Sciences Research Ethics Subcommittee (BDM RESC) at King’s College London (approval number’s LRS-17/18–5510 & LRS-17/18–6465). All participants provided written consent to participate, including children. Parents/guardians provided opt-in or (following an Ethics Committee amendment) opt-out consent in writing, depending on school’s usual practice. Opt-in/opt-out consent was obtained from parents/guardians of all children involved in the study. All children provided written consent. Children were given a verbal introduction to the study, and received an information sheet and consent form that were written in clear and simple language, which was approved by the BDM RESC.

: All participants provided written consent for publications to include extracts from transcripts, and consented to photographs being taken for publication.

: DR and GS are public health professionals from the Borough examined in this study.
