Ethics approval and consent to participate
: This study was approved by the Malagasy Ethics Committee at the Ministry of Health in Madagascar and the Center for the Protection of Human Subjects at the University of California, Davis. To avoid issues with low literacy in the study region, verbal consent from all study participants was obtained. For subjects who were not able to read or write, a witness who was not a study team member was required to be present at the explanation of the study and certify that the participant understood the information and voluntarily agreed to participate in the study or allow for the participation of their child in the study. This consent process was approved by the ethics committees.
