: Separate informed consent for this study (additional to the BSP consent) was not sought from participants because the purpose of this health services research was to test uptake, consent would have been impractical and may itself have reduced participation in the screening. Ethical approval with waiver of consent was received from the NZ Health and Disability Ethics Committee (12/NTA/91).
