Ethics approval and consent to participate

: This study was approved by the Health Sciences and Behavioral Sciences Institutional Review Board at the University of Michigan as well as each of the 14 Adolescent Medicine Treatment Unit (AMTU) institutions that enrolled participants, including: the Human Research Protection Program at the University of South Florida; the Human Subjects Protection Program at the Children’s Hospital Los Angeles; the Office for the Protection of Human Subjects and Institutional Review Board at Children’s National Medical Center; the Committees for the Protection of Human Subjects at the Children’s Hospital of Philadelphia; the Institutional Review Board at the Cook County Health & Hospitals System; the Institutional Review Board at Montefiore Medical Center; the Social/Behavioral Institutional Review Board at Tulane University; the Social and Behavioral Sciences Institutional Review Board at the University of Miami; the Institutional Review Board at St. Jude’s Children’s Research Hospital; the Institutional Review Board at Baylor College of Medicine; the Social, Behavioral, Educational Institutional Review Board at Wayne State University; the Johns Hopkins Medicine Institutional Review Boards; the Fenway Institutional Review Board at the Fenway Institute; and the Colorado Multiple Institutional Review Board at the University of Colorado Denver. All but one AMTU site received permission from their IRB to waive parental consent from participants ages 16 or 17; the site that was not able to waive parental permission did not enroll participants under age 18. Written informed consent was obtained from all TGD youth participants who screened eligible to participate. Written informed consent was waived for provider participants; verbal consent was obtained for all medical and social service provider participants.
