Ethics approval and consent to participate
: The study was approved by the Regional Ethical Review Board at the University of Gothenburg (decision number 714–16). As identification of individual participants was impossible and because there was no contact between respondents and researchers, ethics approval was not required according to Danish and Norwegian legislation [, ]. Participants were adults (at least 18 years old) who gave written informed consent. Participation was voluntary.
