Ethics approval and consent to participate
: Ethical approval has been attained from the University of South Australia Human Research Ethics Committee, Adelaide, Australia (200980), the South Australian Department of Education and Child Development (2008–0055) and the Adelaide Catholic Education Centre (201820). Principals and parents will provide written informed consent, and children will provide verbal assent on each measurement occasion. All ethics committees have approved the consent to participate. All ethical approvals are valid and cover all schools involved in this study.
