Ethics approval and consent to participate
: Ethics approval was sought and obtained through each of the participating hospital’s Institutional Review Boards (Additional fileInternalRef removed: Table S3). A participant information sheet was provided to all caregivers, who were explained that although interviews were conducted within hospital settings, their information would not be shared with the treating doctor or team. It was also explained that the information collected would be de-identified, kept confidential and used for research purposes. Only after the participant felt confident was written informed consent obtained in the local language. Written informed consent was obtained from all participants. All except one participant consented to be audio recorded and notes were taken for this participant.
