Ethics approval and consent to participate
: Ethics approval was given by Jaramogi Oginga Odinga Teaching and Referral Hospital Ethical Review Committee (ERC.1B/VOL.I/263) and The Kenyan National Commission for Science Technology and Innovation. All respondents were informed about the aim of the study and that they could withdraw from participation at any time without consequences. All respondents were asked to provide written informed consent. The consent forms were translated into Kiswahili and Luo languages and then back to English to provide a clear understanding for all respondents as well as ensure an accurate translation. Respondents younger than 18 years were required to provide a signed assent and their tutor’s consent in order to participate. In these cases, parents and legal guardians received information sheets brought home to them by their children and were invited to an open meeting at the school which involved education about the study. The consent forms were signed by the class teacher (tutor) responsible for the participating students, and shortly thereafter, a research assistant collected the forms at the school. The parents and legal guardians gave their oral permission for the tutors to sign the consent forms, this was suggested by the parents due to logistical reasons. No economic compensations were given to any respondent. All research data, regardless of format, were stored securely, and handled according to Kenyan laws and guidelines.
