Ethics approval and consent to participate

: The protocol was reviewed by Ethics Committee of Faculty of Medicine, Universitas Gadjah Mada, Indonesia and obtained research clearance (251/PS-IKM/EC/VII/2017) and research permission (251/PS-IKM/EC/VII/2017). The protocol was further reviewed by REBH and ethical approval certificate (REBH/Approval/2017/053) was obtained from REBH, Ministry of Health, Bhutan. The study was conducted after getting approval from the ethics committee and after obtaining study permission from two study sites JDWNRH and PGH. Written informed consent from individual and parental written consent for children was obtained from the respondents prior to participation. To protect the privacy of the patients the names of the patients were not included in questionnaire. Hospital numbers were not directly recorded, and we have delinked from the registration by using alternative numbers.
