: This study received ethical approval from the College Of Medicine Research and Ethics Committee (COMREC) in Malawi (reference number: P.07/13/1424), and the Liverpool School of Tropical Medicine (LSTM) Ethics Committee (reference number:14.040).Written consent to participate was obtained from all interviewees prior to their interviews.
: The participant information sheet and consent form both included statements about the possibility of publication of anonymised interview content. All participants gave their consent for the publication of material obtained in the interviews by signing or stamping the consent form.
