: The protocol and informed consent forms for the study were reviewed and ethics approval was granted by the Ethics and Scientific Review Committee of the Africa Medical and Research Foundation (AMREF), Nairobi, Kenya (approval number P116/2013). Permission to conduct the study was granted by the County Departments of Health in Nakuru, Kirinyaga and Embu counties and also from the local health administration. All respondents and guardians were informed of the study procedures and voluntarily participated in the study and provided written informed consent.
