: The Thibela TB study was approved by the ethics committees at the University of KwaZulu-Natal and the London School of Hygiene & Tropical Medicine and by the South African Medicines Control Council and the South African Safety in Mines Research Advisory Committee. Miners who participated in the Thibela TB study provided written or witnessed oral informed consent [CitationRef removed]. Approval for the silicosis analysis was subsequently obtained from the University of Kwazulu Natal Biomedical Research Ethics Committee and the Research Ethics Committee of the Faculty of Health Sciences, University of Cape Town.
