Ethics approval and consent to participate
: The study has ethical approval from the Faculty of Medicine and Health Ethical Review Board (ref MREC16–079) and from the Bangladesh Medical Council (ref BMRC/NREC/2016–2019/155). Informed consent to participate in the study was acquired within each component. Participants in interviews and the survey provided written consent. Participants in the focus group discussions provided verbal consent. Approval was granted by the relevant ethical review boards. Data was fully anonymised to ensure confidentiality, and was securely managed.
