Ethics approval and consent to participate
: Ethical approval for this study was obtained from the Massey University Human Ethics Committee (SOA 16/37) and Nepal Health Research Council Ethics Committee (Reg. no. 163/2016) respectively. The participants were clearly informed about the purpose and voluntary nature of the study as well as about the research team using a simple information sheet.
