Ethics approval and consent to participate
: Ethics approval for the 2006–2011 GAPSS and GOSS studies were obtained from the Northern X Regional Ethics Committee of the Ministry of Health (#NTX/05/12/164). Ethics approval for the 2014 GAPSS and GOSS surveys was received from the University of Auckland Human Participant Ethics Committee (#010738). No additional ethics approvals were required in order to access the data for this analysis.
