: Informed consent to participate was obtained in writing for all subjects participating in both phases (or written assent for children under the age of 18 with additional written parental consent, in the Phase 2 study). For both the pilot and full study (Phase 2) we have obtained clearance from the Committee for Protection of Human Subjects at UC Berkeley and the Rwanda National Ethics Committee. The Phase 1 intervention design was determined by the University of Washington Institutional Review Board to not meet the federal definition of “research”, and therefore was not subject to 45 CFR 46.
