Ethics approval and consent to participate
: This study received ethical evaluation and was approved by the Kenya Medical Research Institute, Scientific and Ethical Review Unit (SERU number 2997). The participant consent was done in local languages Kiswahili and Samia. The participants were informed of the purpose of the study, the risk, benefits, confidentiality and information on voluntary participation. All participants provided written informed consent. Additional permissions to conduct the study were provided by the local administration including the chief. The study received additional non-financial support (e.g. access to information) from the Busia County health department after study staff visited the offices.
