Ethics approval and consent to participate
: The study was vetted and approved by NYSHN in keeping with the agreed-upon roles and responsibilities outlined in SMH-NYSHN Research, Data Sharing, and Publication Agreements (See Additional files and ). The study was also approved by the Unity Health Toronto Research Ethics Board (study nos.:15–033 and 15–175). Participants in this study provided written or verbal consent prior to participating in the study, where the option of verbal consent was made available to ensure cultural appropriateness and respect the wishes of participants who would prefer not to provide written consent (for example, due to suspicion, or being uncomfortable with written English). Participants were informed of their right to withdraw from the study at anytime prior to results being disseminated through presentations, reports, and/or manuscripts. We consider consent as an ongoing process and Well Living House and NYSHN researchers shared back analyses and results with the NYSHN membership for review and approval, in person and via email, prior to presenting and/or publishing any materials.
