Ethics approval and consent to participate
: The study received ethical approval from the Bangladesh Medical Research Council (BMRCAIREC/20 I 6–20 I 9 I 250) and from the University of Leeds School of Medicine Research Ethics Committee (MREC16–106). The information sheets were read out to all participants, who were encouraged to ask questions to ensure understanding. Informed written consent was taken from all interview and focus group participants. For the survey, informed written consent was obtained from all participants, this included consent for completing the questionnaire and follow-up at six-months was taken.
: All participants consented to the use of their anonymised data in the analysis and publication of this study. Participants also consented for their anonymous quantitative and qualitative data to be used by other research teams.
