: Study protocol and amendments were approved by the University of Malawi College of Medicine Research and Ethics Committee (COMREC) in Malawi (reference number P.03/12/1192) and the World Health Organisation Human Research Ethics Review Committee (Protocol ID RPC521). Written informed consent was received from adult participants. Written informed consent was also obtained from a parent or guardian for participants under 16 years of age. Participants’ anonymity and confidentiality was strictly maintained. All analyses used de-identified data.
