: Ethical approval for the 2014 GDHS was provided by Ethical Review Committee of Ghana Health Service, Accra, Ghana and the Institutional Review Board of Inner-City Fund International, USA. Written or verbal consent was provided by all participated in the survey. Written informed consent was taken from the parents or guardians of participants under age 16. The authors obtained permission to use the dataset from MEASURE DHS. The DHS Program is consistent with the standards for ensuring the protection of respondents’ privacy. ICF International ensures that the survey complies with the U.S. Department of Health and Human Services regulations for the respect of human subjects.
