Ethics approval and consent to participate
: Institutional Research Ethics Review Board (IRB) of the Arba Minch University, College of Medicine, and Health Sciences provided the ethical clearance. The reference number of the letter was IRB/408/12. Written and signed voluntary informed consent was obtained from all the key informants. The concealment is kept via the use of codes.
