Ethics approval and consent to participate
: Ethical approval was obtained from Advarra Institutional Review Board (IRB) in the U.S., the Excellence in Research Ethics and Science (ERES) Converge IRB in Zambia and the African Medical and Research Foundation (Amref) IRB in Kenya. All participants provided written informed consent. In Zambia, written parental consent was obtained from caregivers of all adolescents under the age of 18. In Kenya, emancipated minors, defined as those who are married, head of the household, pregnant or had children were able to provide written consent on their own behalf, in accordance with national policy []. Written parental consent was obtained with the caregivers of all adolescents under the age of 18 that did not qualify as an emancipated minor.
