: The ATH Study was approved by the Coordinating Ethics Committee of Finnish Institute for Health and Welfare (THL) in 2010 (approval number THL107/6.01.00/2010). Additional administrative permission was not needed to access the data from the ATH study for the current study. Participation was voluntary, and subjects were informed of register linkages. In the current study, register linkages were not used.ATH study data is free to use after you have written a research plan and got the needed variables from ATH study board. We got the right to use data (ExternalRef removed). We used only part of the variables, which are enclosed as supplement in the submission. We asked permission from the member of ATH board to enclose the variables used as supplement and were granted the permission. The data was accessed through administrative permissions at the institution, Finnish Institute for Health and Welfare.
