: The Medical and Health Research Ethics Committee Faculty of Medicine, Public Health and Nursing Universitas Gadjah Mada/Dr. Sardjito General Hospital has approved this study (Ref. KE/FK/0757/EC/2018). All methods were performed in accordance with the relevant guidelines and regulations. Prior to the scheduled interviews, we explained the study objectives and procedures to the participants and asked them to sign an informed consent form in Bahasa Indonesia. For those who were interviewed by phone call, we recorded their oral consent. Except for telephone interviews, all sessions were conducted in private rooms, and participants were given the opportunity to ask questions about the study. For the secondary data, permissions were obtained from each institution in selected provinces. We ensured that individual participants were anonymized in the written reports and publications.
