: The study was approved by the Albert Einstein College of Medicine Office of Human Research Affairs (OHRA) and the human subjects consent for procedures followed were in accordance with the ethical standards of the OHRA and with the Helsinki Declaration of 1975, as revised in 2013. Consent to participate was obtained from the parents/guardians of the minors included in this study (minors are considered anyone under the age of 16) in compliance with the privacy and confidentiality requirements outlined in the <i>Recommendations for the Conduct, Reporting, Editing and Publication of Scholarly Work in Medical Journals</i> [International Committee of Medical Journal Editors (ICMJE) Recommendations)] otherwise known as the Uniform Requirements for Manuscripts (URMs).
