Ethical approval was granted by Ethical Review Board in Stockholm, reference number 2016/32–31. Our manuscript was reviewed by the Arbeintskreis Medizinischer Ethik-Kommissionen In Der Bundesrepublik Deutschland e. V. We received a statement that no additional German ethic approval was needed because there we no legal basis for Ethics Committees to review qualitative research using interviews. The research project complied with German Data Protection Act (BDSG, 20.12.1990) and the Berlin Data Protection Act (BlnDSG, 3.7.1995) to protect the integrity and safety of the participants.Informed written consent was obtained from all individual participants included in the study.
