: The authors assert that all procedures contributing to this work comply with the ethical standards of the relevant national and institutional committees on human experimentation and with the Helsinki Declaration of 1975, as revised in 2008.A self-administered questionnaire piloted in a previous survey was adapted. Informed consent was obtained from survey respondents who completed the pencil and paper questionnaire in privacy and were paid £5 for participation. All methods in this study was approved by Queen Mary University of London ethics committee.
