: This study was approved by Regional Committee for Medical and Health Research Ethics in Central Norway (2019/1064) under the condition of sending an information letter to all participants letting them know the aim of this study and giving them the opportunity to refuse further use of their data. The interviews performed in 2013 and 2015 was also approved by the Regional Committee for Medical and Health Research Ethics in Central Norway (REK nr 2012/1755 and REK nr 2015/188). All participants received both oral and written information so they could make an informed choice about participating. They signed an informed consent form before the interviews started.
