: Ethical review and approval for the study was primarily obtained from the Ethiopian Public Health Institute (EPHI) with a reference number of EPHI 6.13/109. In addition, the regional health bureaus of Amhara, Oromia, Tigray and SNNP provided approval for the study in their regions based on the initial approval from EPHI. Verbal informed consent was obtained from community level participants and signed informed consent was obtained from health workers who participated in the study. Given the high illiteracy rate of community members, verbal consent was deemed sufficient by the ethics approval committee of EPHI. Confidentiality of information was maintained by removing all identifiers.
